要求为儿童接种疫苗的父母和/或法定监护人必须出示原件, current, 在接受免疫接种服务之前，必须持有政府签发的有效身份证件. 

A hard (paper) copy of a complete shot record is required to obtain immunizations at the clinic for children up to 18 years of age; a school note alone is not sufficient. 记录必须翻译成英文，以便在免疫诊所使用. 除英语外的其他语言的免疫接种记录将不被接受.

需要在诊所接种疫苗的儿童可由一名成人陪同. 其他家属将被要求在外面等候.

只有预约才能接种疫苗.
